Admission Process:

 

Requests for service mostly come via the NDIS.

 

A Kyeema representative then makes contact with the participant or nominee and arranges a time to commence the process of intake.

 

Initial intake includes signing of consents, Service Agreements, the development of Schedules of Support, support plan discussion and completion of Goals and Actions form to indicate the agreed supports the participants is purchasing from Kyeema.
