      Ameeyk on Hurd is not open 365 days of the year, and the program is subject to the availability of funding.

      The hours of operation are flexible.  Drop Off and Pick Up times by arrangement with the Respite Supervisor.

      Any arrangements regarding start and finish times, are only to be altered / changed after consultation with the Respite Supervisor.

      Our House programs are on a (4) four monthly planned booking system.

      This means we send out booking information in advance to all of our registered participants.

      Prior to each booking period you will receive a booking calendar on which you indicate your preferences for Respite Dates over the following 4 months. Once everyone’s requests are returned to us we assess them and allocate placement dates according to Participant need, why the Respite is required and the best participant mix. We then send you an individual confirmed booking plan, which has your bookings on it.

      It is not always possible for placements to take place over the weekend and holiday periods, so we suggest that when you return the booking preference form, that you include mid week dates. We make every effort to provide Respite on your nominated dates, however as demand is high, you may not get all of these.

      On occasions you may be asked to change a planned booking to assist a family in crisis.

      We ask for your co-operation in this.

 

Record of Overnight Facility Respite Procedure

This monthly form is used to record the number of nights and hours used by respite Participants. The form is collated for the Quarterly Data Count, required by the Department of Health & Human Services.
