The Breakaway Days are group activities that occur on weekends and include community access, music & dance, stories and outdoor activities. Some of the activities may involve traveling to a venue or another town.
The Breakaway Day Programs have separate sessions for Adults, Adolescents and Children. These sessions are held at different times.
Adults & Children have 2 sessions per month, with Adolescents once per month.
Bookings: The Program staff require advance knowledge of participant numbers to allow for program planning. No participants can attend, unless they have booked in advance.
Programs are co-ordinated with a strict Participant/Staff ratio. Late cancellations must be charged to the participant’s NDIS funding so it is best if Kyeema is notified of cancellations as early as possible.
Adult Breakaway Days 18 years and over
The program is held twice per month from 11:00am – 5:00pm on weekends.
Venue: 50 Lalor Street, Portland.
Time: 11:00am-5:00pm on Saturdays
Cost: As per activity brochure
It is the responsibility of carers to provide transport to and from Lalor Street.
There may be some additional costs, at times. This may be if the group are travelling out of town, or attending the cinema, or purchasing a take away lunch.
Transport pick-up point in Heywood. We assist people who live in Heywood to attend Breakaway Days. Kyeema collects participants from a designated pick-up and drop-off point at the Heywood Bowling Club or a pre-arranged location.
Adolescent Breakaway Days 13-17 years
This program is held once per month.
Venue: 50 Lalor Street, Portland
Time: 11:00am-5:00pm on Sundays
Cost: as per activity

Children’s Breakaway Days 6-12 years
This program is held twice per month.
Venue: 50 Lalor Street, Portland.
Time: 10:00am-4:00pm on Saturdays
Cost: as per activity
Medication
• Participants who need medication administered between 11am and 5pm, must have the correct medication documentation completed CCF-09 Medication Form or CCF-36 Participant Treatment Form. Please complete attached form, listing any food allergies.
• Medication to be supplied in a Webster pack from the pharmacy
• The Webster pack needs to be named and a list of the medications.
• Short Term Medication forms CCF-09 or CCF-36 need to be completed for “over the counter” medications, i.e. Panadol, cough mixtures etc.
