An initial Employment Assistance Plan (Plan) must be prepared for the employee within four months of commencing with a supported employment service. It is done by a staff member who has an understanding of the supported employee’s goals and should be informed by the current wage assessment process.
The intent of this plan is to document Training goals, accommodating the interests of the employee and the requirements of the workplace. The employee’s plan can be reviewed at any time. Subsequent plans occur on an annual basis on their anniversary date.
If the employee does not wish to create a new plan, the plan can be reviewed separately (Employment Assistance Plan SEF-07 - page 3) and documents as such.
A Review of Plan (Employment Assistance Plan SEF-07 - page 3) is part of this process and occurs any time after the initial plan. This can be instigated by the employee or a staff member. The Review of Plan page may be updated or handwritten upon.
The Evidence of Support Provided Form WIF-18 is used to compile evidence of support provided to the individual employee. It covers level of support provided on employment tasks in general and, where applicable, includes assistance provided through the shift on non-work related issues.
