Authorised by Chief Executive Officer

Revised Date: 18 September 2017

Procedure

Kyeema Support Services Inc. is required by law to ensure all hoists and slings are well maintained and in working order.

All hoists are to have a SF-57 Hoist Inspection Register Form located nearby and stored in a folder at Ameeyk House and Lalor St.

All Kyeema Hoists are to be routinely inspected by a trained professional every twelve months. The inspector is to fill out the SF-57 Hoist Inspection Register Form or give a written report to be attached to the form.

Kyeema staff are to visually inspect all hoists prior to use. If the hoist is visibly damaged or there are any concerns as to the safety of its use do not use it and contact the program manager for further instructions.

All slings are to have a SF-58 Participant Sling Check Form stored in a folder at Ameeyk House and Lalor St.

All Kyeema slings are to be routinely inspected by a trained professional every six months.

Kyeema staff are to inspect the sling prior to use. Information on the sling is to be recorded in the relevant SF-58 Participant Sling Check Form and signed off by the inspecting staff member. If a sling is damaged do not use it and contact the Program Manager, select another sling if available and complete the same check before its use.
