Participant Information Forms CCF-40 are electronically forms with a snap-shot of important information about participants. This includes emergency contact information, health and safety information and the individual’s goals.
New participants require a form to be created.
When forms are updated other program areas must be informed by a Management email. If the changed information is important, such as contact details or health and safety issues, the forms should be reprinted in hard copy and replace the version in participant folders in the staff room and the Respite & Independent Living Programs files and/or Day Programs Evacuation backpacks.
