Secondary level school students who have been identified as potential participants for Kyeema’s programs generally benefit from a transition program.
The purpose of a transition program is to support students to feel comfortable and confident in the environment they will move into the following year.
The relevant program manager liaises with the student’s school and in conjunction with the teacher tailors an individualised plan for the student’s transition.
Written plans or agreements should be in place to ensure that the student is covered by the school’s insurance. Transition to a disability organisation is recognised by the Department of Education and Early Childhood Development (DEECD) as a valid pathway for students with disabilities.
Issues to be covered during transition planning include
• Student’s support needs, likes and dislikes
• Information about behavioural issues
• Support staff from the school
• Transportation to and from school
• Times and days of transition
• Options to try different programs during transition
• Contact with parents/carers to ensure they are aware of the plans for the individual
