Carelink+ Shift/Case Notes Procedure

 

Supported employee notes require:

 

      Person first notes – No “we” did this or “I” that did that…e.g., Person’s name was supported 1:1 to transplant seedlings into pots or Person’s name was shown how to do XYZ & was able to complete the task independently with supervision.

      How they were supported - This is VERY important
Note use of verbal prompts or visual demonstration used for instruction & tasks. Did they require redirection back to tasks. Were they able to work independently with supervision or did they require 1:1 support, 1:2 support etc.

      What they did
Use the white board for reference -Transplanting, pricking out, manual work, weeding etc. Don’t forget to reference how they were supported to do these tasks. This is important for audits & NDIS funding review.

      Quantity of work & approximate timeframe
For example, 160 seedlings transplanted in 4 hrs approx.
This gives refence to productivity which is important for wage assessment & tracking progress/decline.

      Identify any issues that are ongoing.
e.g., Behaviours, OHS risks, need for redirection, distraction, reduced capacity or health & wellbeing etc. Identify how many times intervention was required – 3 times, frequently, entire shift etc.

      Strategies or ongoing support used to address issues or behaviours.
What strategy was used to address these issues.

      Note positive outcomes, interactions and goals met.
