This form is sent out every 4 months to all participants who use Short Term Accommodation. The form is completed by the Carers / Participants, listing which dates they request for facility based overnight respite.

 

The form is then returned to the Ameeyk House Coordinator, who collates them, and transfers them to the booking system. These bookings are then confirmed with the Carers/Participants.
