Key Responsibilities

•  Assist, guide and support Support Coordination staff to carry out their job to the best of their ability by clearly defining their role and responsibilities and providing them with adequate resources and training

•  Maintain responsibility for staff recruitment and inductions, in liaison with other relevant managers and CEO

•  Build a trustworthy relationship with participants, carers, families and third parties while maintaining boundaries and privacy

•  Work together with the participant to understand Plan Funding and its purpose, identifying directly what the participant wants to gain from their services and assist them to overcome any challenges that may relate to this

•  Be aware of the potential for conflict of interest, given that Kyeema offers some of the supports people may require, and work always to the participant’s choice and best interests regarding choice of provider

•  Assist the participant to develop knowledge, identify and consider support options, and link the Participant to broader systems of support within the community and surroundings

•  Review and report on participant NDIS plans within the required time frame and provide all necessary information to the relevant parties

•  Help the participant prepare for review and report on achieved participant outcomes

•  Undertake and complete all work in line with both internal and external policies and procedures

•  Ensure all work completed meets the requirements of external auditing agencies

•  Comply with and maintain an up to date knowledge of all relevant NDIS practice standards

•  Ensure all support provided to Participants and aligns with Kyeema’s Vision and Mission statements

 

Child Safe Responsibilities

       Kyeema holds a policy of zero tolerance to racism and has expectations that staff and volunteers will act on incidents of racism

       Kyeema has systems to protect children from abuse, will take all allegations very seriously and will respond to them consistently in line with Kyeema’s policies and procedures

       Kyeema is committed to promoting cultural safety of Aboriginal children, cultural safety of children from culturally and/or linguistically diverse backgrounds and to providing a safe environment for children with a disability

       Kyeema is committed to ensuring the acceptance and inclusion of participants' and workers' gender identities

 

Objective of Role

 

To provide high quality and continuous Support Coordination for NDIS participants and facilitate the implementation of all supports identified in a participants plan while maintaining up to date knowledge of all NDIS practice standards.

 

Tasks:

Conduct staff appraisals of Support Coordination Officers

Annually

Ensure that staff members have clearly defined responsibilities and the resources to carry out their work.

Ongoing

Supervise Support Coordination Staff, including addressing any performance management concerns in a timely fashion

Ongoing

Coordinate and participate in management and staff meetings. Coordinate special staff meetings as required

Monthly or as staff meetings occur

Maintain responsibility for staff recruitment and inductions, in liaison with other relevant managers and the CEO.

Training:

Have input into the development of the agency training plan, noting specific training needs for staff from your Program area.

Annually

In liaison with other Managers develop relevant training opportunities and use designated planning / training days to increase skills development opportunities.

Quality Assurance: re Participants

Ensure all Practice Standards are communicated to Support Coordination staff, and that these standards are met or exceeded.

Quality Assurance:  

Participate in Kyeema’s Continuous Improvement process

Support OHS practices and procedures

Participate in regular program supervision meetings with the CEO

Reporting:

Report relevant programs, projects and activities to Kyeema’s Management Team Meetings. Verbal report to meeting with written report emailed to Office Manager for inclusion in meeting minutes.

Participate in monthly Board Subcommittee meetings if requested to do so by the CEO.

Complete and revise Continuous Quality Improvement documentation when policies, procedures or forms require changing or reviewing

Develop / maintain and update documents relating to work area

Ensure that all record keeping complies with service audits / legislation/standards

General:

Be actively involved in long term planning, with particular emphasis / representation of matters pertaining to own Program area

Oversee input into Kyeema Newsletter and social media of Program area

Budget:

In liaison with CEO and Finance Manager remain informed about the income and expenses of own program area
