Support Planning is an important part of setting out a person’s dreams, goals and ambitions and tracking their achievements. Each participant at Kyeema is required to have an annually reviewed plan.
The Support Plan process starts with the participant and/or nominee engaging in an NDIS Planning Meeting with the Local Area Coordinator. Participants should where possible, choose who is involved in their Support Plan process. At any time during the Support Plan process an individual may seek support from family/carers, staff or an independent Advocate.
The goals from the NDIS plan form the basis for the planning the participant and/or their nominee then does with Kyeema.
The NDIS goals are often broad so the planning process breaks these down to more detailed action items and these are documented on the Goals and Actions form.
During the planning process the Schedule of Supports is created as this documents the hours and cost of the requested services.
A Service Agreement is entered into as well. The Service Agreement authorises Kyeema to allocate funds from the participant’s NDIS package to support their goals.
Each of these forms:
- Goals and Actions
- Schedule of Supports and
- Service Agreement must be signed by a participant or nominee.
A copy of the Goals and Actions Plan should be circulated to all relevant parties involved.
Participant’s goals are tracked through shift notes on Carelink via electronic file notes and are monitored throughout the year.
Employment Assistance Plans are the plans documenting a person’s employment goals and outcomes. These are created annually and should be reviewed within that year as well, around the 6-month mark.
